Probably custom drawn. There have been a lot of custom window bordering applications for XP to add transparency, some of which still work (though barely) in Windows 8. off topic I have been talking to the creator of SIB and his next version will also include being able to have a custom Userpicture frame and native support for changing the start menu theme using the msstyles.
